1. , the undersigned, as the parent or legal guardian of the minor named above (hereafter referred to as "the Participant"), acknowledge, understand, and agree to the following:
2. Inherent Risks: I acknowledge that participation in fitness activities involves inherent risks, including but not limited to muscle strains, sprains, fractures, concussions, or other serious injuries, as well as the risk of severe health complications or death.
3. Voluntary Participation: I affirm that the Participant’s participation in fitness activities provided by Studio Eight Pilates LLC. is entirely voluntary and that I have been informed of the nature and purpose of the activities.
4. Fitness and Medical Condition: I confirm that the Participant is in good physical condition, does not have any known medical conditions that would prevent them from safely participating, and has been cleared by a physician if necessary. I agree to disclose any relevant medical information to Studio Eight Pilates LLC.
5. Release of Liability: I hereby release, waive, and discharge Studio Eight Pilates LLC. , its owners, employees, contractors, and instructors (collectively referred to as "Studio Eight Pilates LLC.”) from any and all liability, claims, demands, or causes of action, including those arising from negligence, that may result in injury, illness, disability, or death during or as a result of participation in fitness activities.
6. Indemnity Clause: I agree to indemnify and hold harmless Studio Eight Pilates LLC. from any and all claims or actions arising out of the Participant’s involvement in fitness activities, including but not limited to claims by third parties caused by the Participant’s actions.
7. Emergency Medical Care: In the event of an emergency, I authorize Studio Eigth Pilates LLC. to seek emergency medical treatment for the Participant. I understand that I am financially responsible for any medical care provided.
